Job summary

Anchor Health in South San Francisco, CA seeks an experienced Hospice RN Case Manager to join our team. You will perform comprehensive patient and family assessments, develop and revise care plans, and coordinate home visits and interdisciplinary care.

The role requires an active California RN license, CPR, and at least two years of nursing experience including hospice/home health. A valid CA driver license and reliable vehicle are also required.

Qualifications

• Graduate of accredited school of nursing.
• Current RN license in California.
• Current CPR Certificate.
• Minimum two years experience as a professional nurse, with at least one year in public, home care or hospice.
• Valid CA driver license and insured vehicle.
• Excellent observation, verbal and written communication skills.

Responsibilities

• Completes initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Performs complete physical assessment and history of current and previous illness.
• Conducts home visits and assesses patient status.
• Initiates and revises the plan of care with patient status and needs.
• Develops care plans with goals based on nursing diagnosis and nursing actions, involving patient and family.
• Administers medications and treatments as prescribed by the physician.
• Attends and participates in Interdisciplinary Group Meetings.
• Maintains and submits accurate clinical notes regarding patient condition and care.
• Supervises ancillary personnel and delegates tasks as required.
