Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to deliver compassionate home-based hospice care.

You will assess needs, develop personalized plans, and lead end-of-life conversations with patients and families, ensuring comfort and symptom control with empathy and clinical excellence. The role requires RN licensure with an accredited nursing degree and 1+ years in acute care; hospice/home health experience is preferred.

Qualifications

• Associates, Bachelors, or Masters in nursing from an accredited program.
• 1+ years of recent acute care experience; hospice/home health experience preferred.
• Complete initial and ongoing assessments to determine hospice needs.
• Develop individualized care plans with patient and family.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
• Must have reliable transportation, valid driver’s license, and proof of auto insurance.

Responsibilities

• Assess patient needs and develop comprehensive hospice plans with the care team.
• Deliver individualized nursing care in patients' homes with clinical excellence.
• Educate patients and families on disease process, medications, and symptom management.
• Collaborate with interdisciplinary team to coordinate transitions and on-call coverage.
