Job summary

The Care Team Hospice is seeking a compassionate Hospice RN Case Manager to join our team in Decatur, IL. You will coordinate patient care, manage individualized plans of care, and collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort, dignity and quality of life for patients at home or in our hospice setting.

Active RN license is required; hospice experience and familiarity with Homecare Homebase (HCHB) are preferred.

Qualifications

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards and individualized plans of care.
• Complete scheduled visits, admissions, and clinical interventions with accurate documentation in Homecare Homebase (HCHB).
• Respond to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exemplary patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team to coordinate care and support positive outcomes.
• Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care.
