You will conduct assessments, develop care plans, and ensure accurate documentation while engaging with families and physicians to deliver patient-centered hospice care.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited School of Nursing.
• 1+ year hospice, home health, or related experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and ability to work independently in the field.
• Excellent communication across care teams and families.

Responsibilities

• Conduct comprehensive, ongoing assessments to understand each patient's evolving needs.
• Build and adjust personalized care plans with physicians and care teams.
• Administer medications and treatments per physician orders.
• Maintain accurate, timely clinical documentation.
• Serve as the communication bridge between families, physicians, and community health partners.
• Educate patients and caregivers to ensure confidence and support.
• Participate in on-call rotation per team policy.
