Transitions Hospice is seeking an RN Case Manager to serve as the clinical anchor for patients and families in Mattoon/Charleston and surrounding areas. You will coordinate care, guide the interdisciplinary team, and respond when it matters most.

This role requires an active RN license in Illinois, graduation from an accredited nursing program, and 1+ year hospice or home health experience. Expect a schedule from 8 AM to 8 PM with rotating 4-day and 3-day weeks, including weekends, in a

• Graduate of an accredited school of Nursing.

Responsibilities

• Coordinate patient care and guide the interdisciplinary team.
• Conduct comprehensive and ongoing patient assessments.
• Administer medications and treatments per physician orders.
• Maintain accurate and timely clinical documentation.
• Serve as the communication bridge between families, physicians, and care partners.
• Educate patients and caregivers to support informed decisions.
• Participate in on-call rotation per team policy.
