Job summary

Shalom Hospice in Tennessee is seeking a compassionate Registered Nurse to plan, organize and direct home-based hospice care. The RN will perform comprehensive patient assessments, develop and revise care plans, administer prescribed treatments, and educate patients and families on pain management and self-care.

As a key member of the interdisciplinary team, you will coordinate care, participate in on-call coverage, and ensure high standards of comfort and quality for patients and their families

Qualifications

• Graduate of an accredited school of nursing.
• RN licensed with good standing.
• 1–2 years public health/home care/hospice experience preferred.
• Must have valid driver’s license and insured vehicle.
• CPR certification required.

Responsibilities

• Perform initial and ongoing patient/family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise care plans in collaboration with the care team.
• Counsel patients and families; educate on self-care.
• Act as case manager for assigned caseload.
• Participate in on-call duties and coordinate equipment.
