Cabrillo Hospice is seeking an experienced Hospice RN Case Manager to oversee patient care, mentor interdisciplinary teams, and ensure compliance with clinical best practices. The role emphasizes leadership, case management, and regulatory compliance in a dynamic home-based setting.

You will travel to multiple locations, supervise care plans, and participate in quality improvements to enhance patient outcomes. Valid RN license and Medicare knowledge are required.

Qualifications

• Bachelor’s degree or higher in a health-related field.
• RN license in the state of practice.
• Knowledge of Medicare Conditions of Participation and regulatory standards.
• Proficiency with Outlook, Word, and Excel.

Responsabilités

• Lead hospice care team, mentor staff, and maintain high standards of care.
• Provide direction to interdisciplinary team members and oversee patient care plans.
• Oversee case management, care plans, and patient outcomes.
• Ensure regulatory compliance with Medicare and hospice standards.
• Review care plans in EMR for accuracy and adherence to policies.
• Participate in quality assurance initiatives and team meetings.
• Lead on-call leadership and 24/7 oversight of patient care.
