Job summary

The RN, Case Manager at Chapters Health System assesses patient and family needs, coordinates the Plan of Care with the IDT, and delivers clinical, palliative and supportive care to keep patients in their homes as long as possible.

This role requires Florida RN licensure, valid driver’s license, and excellent communication skills; it includes field visits, documentation in EMR, and collaboration with physicians and caregivers in a dynamic hospice setting.

Qualifications

• Current RN license in the state where the employee will be working.
• Minimum of one year nursing experience in hospice or hospital setting preferred.
• Active BLS certification for healthcare professionals from the American Heart Association or Red Cross.
• Florida Department of Elder Affairs Alzheimer’s Disease and Related Dementias Training for PACE locations if applicable.
• Experience using an EMR/EHR system.

Responsibilities

• Provides and manages direct care to patients and families as part of the Interdisciplinary Team (IDT).
• Coordinates the Plan of Care reflecting evolving patient needs.
• Educates patient, family, caregivers and other health professionals about disease, palliative interventions and safety practices.
