Job summary

Chapters Health System seeks a Registered Nurse, Case Manager to assess patient and family needs, coordinate care, and deliver clinical and palliative services in patients' homes in Florida.

The role requires current RN license in the state, 1+ year hospice or hospital experience, and active BLS. The RN will work with the Interdisciplinary Team and document in the EMR while ensuring timely communication and safety.

Qualifications

• Current license as RN in the state where the employee will be working.
• Minimum of one (1) year nursing experience in hospice or hospital experience preferred.
• Active BLS for healthcare professionals from American Heart Association or Red Cross.

Responsibilities

• Assess and identify patient/family needs using the nursing process.
• Coordinate Plan of Care with Interdisciplinary Team (IDT).
• Provide clinical, palliative and supportive care to patients in their home.
• Complete initial and semi-annual assessments for all services.
• Document in EMR and communicate with IDT using SBAR.
