Job summary

Elara Caring is seeking a Hospice Registered Nurse Case Manager in Sallisaw, OK. You will assess hospice patients and families, monitor and communicate changes, and educate staff and families on the plan of care and hospice principles.

The role requires an Oklahoma RN license, 1+ year of clinical nursing experience, and the ability to lift up to 50 pounds. A driver's license or reliable transport is required to travel to patient homes.

Qualifications

• Active RN license in the state of Oklahoma.
• 1+ year of nursing experience in a clinical care setting.
• Must be able to lift, push, pull and transfer patients up to 50 pounds.
• Must possess a valid driver's license or reliable transportation.

Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and promptly communicate significant changes to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, and other care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
