Job summary

Arkansas Hospice, Inc. is seeking a Registered Nurse Case Manager to oversee and participate in the provision of nursing interventions for an assigned hospice caseload. The role emphasizes coordinating care, communicating with families and providers, and maintaining documentation in line with policy.

Qualifications

• Graduation from an accredited school of nursing; BSN preferred.
• Minimum of one year nursing experience in hospital, home health, hospice, or related program.
• Licensed RN in Arkansas.
• Valid Arkansas driver’s license and auto insurance required.

Responsibilities

• Manages patient care after initial admission and assignment to a designated RN Case Manager.
• Assists in the admission process as requested.
• Interprets Arkansas Hospice, Inc. policies for nursing staff to ensure understanding and compliance.
• Coordinates with interdisciplinary team for the plan of care on an ongoing basis.
