Job summary

Anchor Health is seeking a compassionate Hospice RN Case Manager to coordinate patient care at home and in community settings. The role emphasizes comprehensive pain and symptom management, education for families, and collaboration with an interdisciplinary team.

The ideal candidate holds a CA RN license, has hospice experience (preferred), CPR certification, and reliable transportation to visit patients at home.

Qualifications

• Must hold a valid CA RN license.
• Hospice experience preferred, at least 1 year.
• Current CPR certification.
• Valid driver’s license with reliable transportation.
• Excellent interpersonal skills and passion for hospice.

Responsibilities

• Provides care to patients with terminal illnesses and coordinates the hospice care plan.
• Performs prescribed nursing treatments, administers medications, and manages pain/symptoms.
• Documents care, revises plans of care as needed, and communicates with physicians and families.
• Acts as Case Manager and coordinates patient care with the interdisciplinary team.
• Educates patients and families on self-care, medications, and safety at home.
• Ensures a safe home environment and continuity of care across care settings.
