• Perform scheduled home visits, teach caregivers, and provide patient/family support.
• Maintain up-to-date patient records and document changes in the plan of care.
• Coordinate communication with the Interdisciplinary Team and attending physicians.
• Attend Interdisciplinary Team meetings and ensure continuity of care across settings.
• Provide 24/7 on-call coverage as needed and participate in patient death events.
