The role requires a CA RN license, CPR certification, and a BSN preferred; it involves home visits, collaboration with the interdisciplinary team, documentation, and scheduling flexibility to cover 24/7 on-call as needed.

Formación

• Graduation from an accredited school of nursing.
• Current California RN license; CPR certification.
• California driver’s license; timely renewal required.

Responsabilidades

• Admissions of new patients and evaluation for hospice services.
• Perform scheduled home visits, teach caregivers, and provide patient/family support.
• Maintain up-to-date patient records and document changes in the plan of care.
• Coordinate communication with the Interdisciplinary Team and attending physicians.
• Attend Interdisciplinary Team meetings and ensure continuity of care across settings.
• Provide 24/7 on-call coverage as needed and participate in patient death events.
