Transitions Hospice in Marshall, IL is seeking an RN Case Manager to coordinate care, lead the interdisciplinary team, and support families in end‑of‑life care. You will conduct assessments, develop care plans with physicians, administer treatments, and ensure accurate documentation.

The role requires an active RN license, nursing degree, and hospice experience; strong communication and independent clinical judgment are essential.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• 1+ year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and the ability to work independently in the field.
• Excellent communication skills across care teams and families.

Responsibilities

• Conducting comprehensive, ongoing assessments to understand each patient's evolving needs
• Building and adjusting personalized care plans in collaboration with physicians and care teams
• Administering medications and treatments per physician orders
• Keeping clinical documentation accurate, timely, and meaningful
• Serving as the communication bridge between families, physicians, and community health partners
• Educating patients and caregivers so they feel confident and supported
• Participating in on-call rotation per team policy
