Good Shepherd Hospice is seeking a compassionate Registered Nurse Case Manager to serve patients and families in Marshall and surrounding communities. The RN provides nursing assessment, planning and care to maximize the comfort and health of patients and families.

The RN responds to all phone calls promptly and appropriately, makes patient visits as necessary and completes after hours admissions and deaths as needed.

Formación

• Registered Nurse licensed in the state where care is provided.
• Experience in hospice, home health, geriatrics or general medical/surgical nursing.
• Commitment to the hospice philosophy of care.

Responsabilidades

• Receives patient assignments and coordinates care with physicians and staff.
• Maintains regular communication with attending physicians and families.
• Assesses physical, psychological, social and spiritual factors for the Plan of Care.
• Maintains up-to-date patient records and Medicare paperwork.
• Instructs patient and caregiver on medications, personal care and safety.
• Provides 24/7 coverage to patients and families as needed.
• Evaluates hospice aides and LPN regularly.
• Participates in team meetings and care plan conferences.
• Provides support at the time of death.
