Job summary

Hospice of Southern Illinois is seeking a Staff Registered Nurse to assess patient and family needs, plan and implement care, and evaluate outcomes as part of the interdisciplinary team. The role includes direct patient nursing and after-hours on-call duties, with travel within Belleville service area and 40 hours per week.

Qualifications

• Assess patient/family/caregiver needs and plan care.
• Provide direct patient care and participate in on-call care.
• Maintain timely medication orders and document in IDT.

Responsibilities

• Maintain a caseload of patients.
• Oversee the direction and coordination of patient care for the patient and family.
• Provide direct nursing care including participation in on-call after-hours care.
• Work on-call in the absence of the on-call nurse.
• Demonstrate required registered nurse clinical skills.
• On each visit complete a physical assessment, medication review, and symptom/pain evaluation.
• Communicate findings to hospice physician and obtain orders to manage care.
• Timely ordering of medications from pharmacy.
• Communicate pertinent information to all disciplines by communicator, telephone, encrypted text messaging, and at the IDT meeting.
• Review and update the Plan of Care (POC) on each visit.
