Job summary

Allay Hospice is seeking a Hospice RN Case Manager to provide patient-centered care with a focus on meaningful, unrushed hospice support. You will benefit from a low patient-to-nurse ratio, strong interdisciplinary team collaboration, and a dedicated after-hours triage team, using the Homecare Homebase EMR for streamlined documentation.

The role requires a RN license and prior hospice or palliative care experience; travel within the territory is expected.

Qualifications

• Graduate of an accredited nursing program.
• Current RN license in state of operation.
• Hospice or palliative care experience preferred.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

Responsibilities

As a RN Case Manager You will:

Qualifications

• • Graduate of an accredited nursing school with current licensure to practice in state of operation
  • One year nursing experience required, 3+ years preferred
  • Experience in hospice or a similar setting preferred
  • Knowledge of the hospice philosophy of care
  • Commitment to clinical and documentation excellence
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
