Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license and access to a dependable vehicle.
• 1+ years of clinical experience preferred.

Responsibilities

• Make initial nursing evaluation within 48 hours of referral.
• Identify patient's needs and re-assess every 15 days.
• Coordinate and document the plan of care.
• Collaborate with patient/family and healthcare team.
