Job summary

BAYADA Home Health Care in Marmora, NJ is seeking a Full-Time Registered Nurse (RN) Case Manager, Hospice to join our hospice team. The RN will perform home visits, coordinate care, and support clients in their homes with compassionate, patient-centered care.

The role requires an active NJ RN license and at least 1 year of clinical experience; home care or hospice background is preferred. Travel is required to Atlantic and Cape May counties. BAYADA offers comprehensive benefits and career growth.

Qualifications

• Current NJRN license.
• Minimum 1 year of recent clinical experience.
• Home care and/or hospice experience preferred but not required.
• Proficient with EMR systems.
• Ability to travel to cases as assigned.

Responsibilities

• Making home visits to clients in designated geographic territories.
• Performing duties including administration of medications, wound care, treatments, and comfort care.
• Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.
• Following up with, executing, and documenting doctors' orders.
• Performing client assessments as necessary.
• Occasional on-call after hours and/or weekends.
