Job summary

Copper Sky Hospice is seeking a compassionate Registered Nurse Case Manager to provide high-quality hospice care in patients' homes, coordinating with an interdisciplinary team to optimize clinical outcomes and quality of life.

The role requires an active Arizona RN license (or compact state), BLS certification, and at least one year of professional nursing experience. Travel is involved to patient homes, with documentation completed within 24 hours of visits.

Qualifications

• Graduate of an accredited school of nursing; BSN preferred.
• Minimum of one year of professional nursing experience required.
• Arizona RN license or compact state.
• Current BLS certification from a recognized provider.

Responsibilities

• Conduct initial and ongoing assessments of patients and families to determine hospice care needs.
• Provide skilled nursing care in the home setting using the nursing process.
• Develop, implement, and update individualized care plans based on patient assessments.
• Coordinate and deliver interdisciplinary hospice services with the care team.
• Complete accurate and timely documentation within 24 hours of each patient visit.
