Copper Sky Hospice in Phoenix seeks a Registered Nurse Case Manager to provide high-quality patient care in the home hospice setting, following physician orders and company policies. In this role, you will collaborate with an interdisciplinary care team to support patients and families while ensuring the best possible clinical outcomes and quality of life.

You will develop, implement, and update individualized care plans, coordinate hospice services, and document visits within 24 hours.

Qualifications

• Graduate of an accredited school of nursing; BSN preferred.
• Minimum of one year of professional nursing experience.
• Hospice or home health experience preferred.
• Experience caring for aging or palliative patients desirable.

Responsabilités

• Assess patients and families to determine hospice care needs.
• Provide skilled nursing care in patients' homes.
• Develop and update individualized care plans.
• Coordinate interdisciplinary hospice services with the care team.
• Document visits within 24 hours.
• Administer medications as prescribed.
• Educate patients and families on care techniques and symptom management.
• Serve as primary Case Manager and manage caseloads.
• Communicate status and changes with physicians and team.
