Gentiva Hospice is hiring a skilled RN Case Manager to deliver compassionate home-based hospice nursing across diverse patients and families. You will perform comprehensive assessments, develop care plans, coordinate with IDG, and provide education on end-of-life care and symptom management.

The role emphasizes interdisciplinary collaboration, regulatory compliance, and timely documentation. A valid RN license and CPR certification are required, with hospice experience preferred.

Qualifications

• RN with current license in the state of employment.
• Hospice or palliative care experience preferred.
• Strong assessment and patient education skills.
• Ability to work with interdisciplinary teams and document care plans.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic RN assessments.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized care plans with visit frequency and risk identification.
• Coordinate care across interdisciplinary teams and document timely records.
