Job summary

The Care Team Home Health & Hospice is seeking a Registered Nurse Case Manager to provide skilled nursing care and coordinated end-of-life support to patients in homes and facility settings. You will collaborate closely with the interdisciplinary team to ensure quality patient care in a compassionate environment.

Standard hours are Monday–Friday, 8:00 AM–4:30 PM, with a 1-hour surrounding radius around Bethlehem, PA.

Qualifications

• Active Registered Nurse (RN) license required.
• Hospice or home health experience preferred.
• Strong clinical assessment and care coordination skills.
• Ability to work independently in the field and in homes.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized plans of care.
• Provide skilled nursing care in accordance with hospice standards and physician orders.
• Coordinate care with physicians, caregivers, and interdisciplinary team members.
• Educate patients and families on symptom management, disease progression, and end-of-life care.
• Document assessments, interventions, and care plans in the EMR HCHB.
• Participate in interdisciplinary team meetings and care planning.
• Support after-hours patient needs as part of an on-call rotation when applicable.
