Job summary

The Care Team Hospice in Bethlehem, PA is seeking a Hospice Registered Nurse Case Manager (RNCM) to provide skilled nursing care and coordinate end-of-life support for patients in their homes and facility settings, collaborating closely with the interdisciplinary team. Schedule is Monday–Friday, 8:00 AM–4:30 PM with Bethlehem, PA and up to a 1-hour surrounding radius.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized plans of care
• Provide skilled nursing care in accordance with hospice standards and physician orders
• Coordinate care with physicians, caregivers, and interdisciplinary team members
• Educate patients and families on symptom management, disease progression, and end-of-life care
• Document assessments, interventions, and care plans accurately and in a timely manner in the EMR HCHB (Homecare Homebase)
• Participate in interdisciplinary team meetings and care planning
• Support after-hours patient needs as part of an on-call rotation when applicable

Schedule, Hours & Territory

• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
• Bethlehem, PA, and up to a 1-hour surrounding radius
• RN Case Managers may participate in an on-call rotation with additional pay
• Some branches offer dedicated on-call staff, resulting in backup on-call coverage only
