Job summary

Lehigh Valley Health Network is seeking a Hospice Case Manager (RN) to coordinate and direct home health care for a defined caseload in Bethlehem/Hellertown and surrounding areas. The role emphasizes interprofessional collaboration and adherence to patient care standards.

Responsibilities include assessing patient needs, developing care plans with the physician and team, and educating patients and families to achieve goals and quality outcomes.

Qualifications

• Specialized NLN approved nursing program.
• 1 year recent experience providing direct care in a healthcare setting.
• Knowledge of patient education techniques and principles.
• Ability to take initiative and function independently without direct supervision.
• Ability to actively participate as a member of a care team.
• Excellent time management and organizational skills.
• Ability to manage a caseload of patients as per agency standards.
• Ability to travel to patient care assignments.
• Knowledge of Conditions of Participation and CMS data collection requirements.

Responsibilities

• Coordinate and direct delivery of home care for an assigned caseload in collaboration with the interdisciplinary team.
• Develop, review, and revise the plan of care to meet patient goals and quality metrics.
• Collaborate with physicians and other network entities for care progression.
• Provide patient and caregiver education on end-of-life, palliative care, and discharge planning.
• Promote patient/caregiver autonomy and assess changes in patient status.
