Job summary

The Care Team Hospice is seeking a Registered Nurse Case Manager to provide skilled nursing care and coordinate end-of-life support for patients in home and facility settings, collaborating with the interdisciplinary team.

Based in Bethlehem, PA (Bethlehem Hospice), the role follows standard hours Monday–Friday, 8:00 AM to 4:30 PM, with some on-call rotation and travel within a 1-hour surrounding radius. Active RN license required; hospice/home health experience preferred.

Qualifications

• Active RN license and hospice/home health experience preferred.
• Strong clinical assessment and coordination skills.
• Ability to work independently in the field.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized plans of care.
• Provide skilled nursing care in accordance with hospice standards and physician orders.
• Coordinate care with physicians, caregivers, and interdisciplinary team members.
• Educate patients and families on symptom management, disease progression, and end-of-life care.
• Document assessments, interventions, and care plans accurately in the EMR HCHB.
• Participate in interdisciplinary team meetings and care planning.
• Support after-hours patient needs as part of an on-call rotation when applicable.

The Care Team Hospice is seeking a Registered Nurse Case Manager to provide skilled nursing care and coordinated end-of-life support to patients in their homes and facility settings while collaborating closely with the interdisciplinary team.

Schedule, Hours & Territory

• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
• Bethlehem, PA, and up to a 1-hour surrounding radius
• RN Case Managers may participate in an on-call rotation with additional pay
• Some branches offer dedicated on-call staff, resulting in backup on-call coverage only

Qualifications

• Active Registered Nurse (RN) license
• Hospice or home health experience preferred
• Strong clinical assessment and care coordination skills
• Ability to work independently in the field
• Strong communication and documentation skills
• Experience using HCHB (Homecare Homebase) preferred

Field Role Requirements

• Active driver’s license
• Ability to pass a criminal background check
• Ability to pass a Motor Vehicle Record (MVR) check
• Current TB testing in accordance with agency policy
• Current CPR certification as required for the role
