Job summary

Covenant Retirement Communities & Covenant Care is hiring a Hospice RN, Case Manager to plan, organize, and direct hospice services within patients' homes and communities in California. The role emphasizes community health education and family involvement in care planning.

You will assess needs, develop care plans, and collaborate with an interdisciplinary team to provide comprehensive hospice care, including medication administration and family counseling.

Qualifications

• Associate degree in Nursing is required.
• Bachelor’s degree in Nursing is preferred.
• RN license and CPR certification are preferred where state requirements apply.
• Hospice care experience is preferred.

Responsibilities

• Completes an initial assessment of patient and family to determine hospice care needs.
• Provides a complete physical assessment and history of current and previous illness.
• Regularly re-evaluates patient nursing needs and revises the plan of care as needed.
• Communicates updates to the team and patient family; participates in IDT meetings.
• Develops care plans based on nursing diagnosis and patient goals.
• Administers medications and treatments as prescribed by the physician.
• Counsels patient and family in meeting nursing and related needs.
• Provides health care instructions per assessment and plan of care.
