Job summary

Anchor Hospice and Palliative Care is seeking an experienced RN Case Manager in California to administer nursing care per the plan for terminally ill patients. You will work within a compassionate team to support patients and families through transitions and ensure dignified care.

The role requires CA RN licensure, CPR certification, and effective communication with physicians and the care team to develop and execute an interdisciplinary care plan focused on comfort and quality of life.

Qualifications

• Licensed Registered Nurse in California in good standing.
• Hospice experience preferred.
• Current CPR certification.
• Valid driver’s license and automobile insurance.

Responsibilities

• Organize patient care and develop interdisciplinary plan of care.
• Provide comprehensive nursing assessments and palliative interventions.
• Assess and implement pain and symptom management to improve quality of life.
