Responsibilities

• Provide professional nursing care using the nursing process.
• Assess and evaluate patient status.
• Initiate and revise the plan of care with patient/family involvement.
• Coordinate patient care as case manager when assigned.
• Communicate with physicians and community health professionals.
• Maintain accurate clinical notes and document pain/symptom changes.
