Job summary

Stillwater Hospice in Billings, Montana is seeking an experienced Hospice Registered Nurse Case Manager. You will plan, organize, and direct home-based hospice care, coordinating with an interdisciplinary team to meet patient and family needs.

The role requires 1–2 years in acute care and 1–2 years in home care, current state RN licensure, and driving as essential. A bachelor’s degree is preferred; on-call duties and learning development are available.

Qualifications

• Graduate of an accredited school of nursing.
• One to two years of recent acute care experience.
• One to two years of recent home care experience.
• Current licensure in state.
• Must be a licensed driver with insured vehicle.
• Bachelor’s degree, with hospice care experience preferred.
• Management experience not required.

Responsibilities

• Performs comprehensive patient and family assessments to determine hospice needs.
• Develops and updates the plan of care in collaboration with the interdisciplinary team.
• Delivers professional nursing care and administers prescribed medications.
• Coordinates patient care as Case Manager to meet caseload needs.
• Teaches patient and family self-care techniques and health maintenance.
• Participates in on-call duties and ensures safe care environments.
