Job summary

Hospice of Missoula is seeking a Registered Nurse Case Manager to oversee patient and family care plans, supervise CNAs and home makers, and coordinate with the Hospice team and physicians. You will ensure accurate documentation and participate in call coverage as needed.

The role requires Montana RN licensure, a valid driver's license, and a minimum of one year in hospice or home health preferred. Strong interpersonal and organizational skills are essential.

Qualifications

• Graduate of an accredited Registered Nursing Program.
• Maintains current Montana RN license.
• Minimum of one year hospice or home health experience preferred.
• Valid current Driver’s License required.
• Furnish own transportation.

Responsibilities

• Perform new patient intake assessments across physical, psychological, social, and spiritual parameters.
• Develop or assist with comprehensive plan of care for patients.
• Coordinate care with Hospice team and families; supervise staff as needed.
• Provide direct patient care within Montana RN practice guidelines.
• Participate in bereavement follow-up per policy.
• Attend volunteer trainings and community education events.
