Job summary

Choice Health at Home is seeking a Hospice RN Case Manager to deliver compassionate, patient-centered care in home settings. You will assess needs, coordinate plans of care, and support families throughout the end-of-life journey.

The role requires an active RN license, CPR certification, and a valid driver’s license; hospice experience is preferred and HomeCare HomeBase familiarity is a plus. This is a full-time position in Greenwood Village, CO.

Qualifications

• Active Registered Nurse license required.
• CPR certification required.
• Valid driver’s license required.
• Valid auto insurance required.
• At least 2 years Hospice RN experience preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform initial and ongoing hospice assessments and document findings.
• Develop and update the plan of care to ensure quality and continuity.
• Educate patients and families on disease process, end-of-life care, and self-care.
• Coordinate with the IDG and other staff to tailor care.
• Provide on-call coverage and respond to emergencies as assigned.
• Inform physicians and monitor patient outcomes.
