Job summary

Interim HealthCare Hospice & Palliative Care in Indiana seeks a Hospice Registered Nurse Case Manager (RN) to coordinate patient care, ensuring comfort and dignity for our clients. You will work with a multi-disciplinary team to implement and adjust care plans as patient needs change.

Qualified candidates hold an ADN or AAS (BSN preferred), valid RN licensure in Indiana, CPR/FA certification, reliable transportation, and at least one year of hospice or similar experience.

Qualifications

• ADN or AAS required; BSN preferred.
• RN license in the state where the position is located.
• Current CPR/FA certification.
• Reliable transportation to/from care sites and/or work locations.
• One year of RN hospice or similar experience preferred.

Responsibilities

• Oversee the implementation and ongoing assessment of the patient's plan of care with the care team.
• Develop and implement a comprehensive care plan addressing patient needs.
• Communicate patient conditions and collaborate with providers as needs evolve.
