Job summary

Jobot is seeking an experienced RN Case Manager to provide hospice nursing care in patients' homes and facilities. You will develop individualized care plans, coordinate with the care team, and educate families on end-of-life care.

Qualifications

• Active RN license in the state of practice.

Responsibilities

• Provide nursing care to patients in hospice care in patient homes and facilities.
• Develop and implement individualized care plans for patients and their families.
• Coordinate with interdisciplinary team members to provide comprehensive care to patients.
• Administer medications and treatments as prescribed.
• Monitor and assess patient symptoms and provide comfort measures as needed.
• Educate patients and their families on end-of-life care and support.
• Document patient care and progress in electronic medical records.
• Provide emotional and spiritual support to patients and their families.
• Advocate for patients and their families to ensure that their needs are met.
• Participate in on-call rotation as needed.
