Job summary

Stillwater Hospice is seeking an RN Case Manager to deliver high-quality nursing care to patients in their homes across Kosciusko, LaGrange, Noble, Whitley, DeKalb counties and parts of Allen and Wabash Counties. The role supports the hospice philosophy, coordinates interdisciplinary care, and ensures compassionate patient-family engagement.

Graduate of an accredited nursing program with Indiana RN license and CPR certification is required or obtained during orientation.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license to practice in Indiana.
• Bachelor’s degree in nursing preferred.
• CPR certification required or within orientation period.
• One year of recent hospital or home health care experience preferred.
• Must have a current motor vehicle operator’s license and access to an automobile with insurance.
• Must be able to drive independently to patient homes.
• Must perform essential nursing duties safely.

Responsibilities

• Provide nursing care to hospice patients in their homes.
• Assess, plan, implement, and revise patient care plans.
• Document findings and communicate with the care team.
• Supervise LPNs, Home Health Aides, CNAs, and other staff as needed.
• Participate in on-call and holiday rotations.
• Coordinate care with physicians and other providers.
