Mount Carmel Home Care & Hospice in Columbus, OH is seeking a compassionate Hospice RN Case Manager to coordinate care for hospice patients and families. You will develop individualized care plans, manage symptoms, and collaborate with an interdisciplinary team to ensure comfort and dignity at end of life.

Qualifications

• Graduate of an accredited nursing program and active Ohio RN license.
• Current CPR certification.
• Minimum 1 year of acute care nursing experience.
• Valid driver's license and reliable transportation.
• Excellent communication, assessment, and critical thinking skills.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized plans of care.
• Coordinate care with physicians, social workers, chaplains, aides, and team members.
• Manage pain and symptoms using hospice practices.
• Provide education and support to patients, families, and caregivers.
• Monitor patient status and communicate changes to the team.
• Participate in IDG/IDT meetings and coordinate care planning.
• Ensure accurate documentation in the electronic medical record.
• Supervise Hospice Aides and LPNs to ensure quality care delivery.
• Promote dignity, comfort, and quality of life for patients and families.
