Job summary

LifeCare Home Health Family seeks an RN Case Manager to oversee admissions and provide nursing care to patients at home or hospice locations in Georgia. You will coordinate with the Interdisciplinary Team, patients, families, and referring agencies to ensure continuity of care.

The role requires two years of RN clinical experience and one year hospice, with active RN licensure and a valid driver's license. Expect travel and participation in QAPI and on-call rotations.

Qualifications

• Two (2) years experience as a Registered Nurse in a clinical care setting.
• One (1) year full time hospice experience.
• State licensure as a Registered Nurse; valid driver's license.

Responsibilities

• Assess hospice patients and identify physical, psycho-social, and environmental needs.
• Develop and document the plan of care in correlation with the assessment.
• Provide nursing care with infection control measures (OSHA).
• Collaborate with Interdisciplinary Group/Team to meet patient/family needs.
• Communicate with attending and hospice physicians involved in care.
• Submit required paperwork timely.
• Participate in hospice-sponsored in-service programs and QAPI participation.
• Promote hospice philosophy and policies.
• Meet productivity standards.
• Perform on-call duties and provide on-call services as assigned.
