Job summary

Affinity Health Management is seeking a skilled Registered Nurse to lead hospice care in the Bethlehem area. The role focuses on comprehensive patient and family assessments, planning and directing home- and community-based hospice services.

The nurse will provide direct patient care, develop and revise care plans, administer prescribed medications, and educate patients and families while coordinating with the interdisciplinary team. On‑call duties are involved.

Qualifications

• Graduate of an accredited school of nursing.
• RN license in good standing to practice.
• 1 year of professional nursing experience within the last 3 years preferred.
• CPR certification required.

Responsibilities

• Conduct initial and ongoing patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise the plan of care with patient and family.
• Administer medications and treatments as prescribed by the physician.
• Counsel patient and family in nursing and related needs.
• Maintain accurate clinical notes regarding patient condition and care.
• Communicate with physicians and care team regarding patient needs.
• Participate in on‑call duties and ensure equipment and services are available.
