Job summary

Responsibilities

• Provides end-of-life care in patient’s home as directed.
• Delivers medications as prescribed and documents treatments.
• Ensures proper admissions, transfers, and discharges; maintains chart accuracy.
• Participates in on-call coverage and case conferences.
