Job summary

Alternate Solutions Health Network seeks a Registered Nurse Case Manager for Hospice to lead patient assessments, plan of care, and coordination of multidisciplinary teams in the Akron area.

You will conduct skilled visits, update orders, monitor patient condition, and ensure compliant documentation while delivering compassionate end-of-life care in home settings. A valid RN license and hospice experience preferred.

Qualifications

• Registered nurse with current state license and hospice experience preferred.
• Ability to coordinate multidisciplinary care and document outcomes.
• Professional presence and patient-/family-centered approach.

Responsibilities

• Oversees end-of-life patient care from start to finish.
• Conducts initial and ongoing skilled nurse visits as per plan of care.
• Updates doctor orders and communicates changes to the team.
• Coordinates care with physicians, aides, and other staff.
• Documents all treatments and education per Medicare guidelines.
