Job summary

Paycom - ATS is seeking a Registered Nurse to deliver compassionate in-home hospice care across ages. You will perform initial and ongoing assessments, develop and adjust plans of care, and educate patients and families to support comfort and dignity.

You will coordinate with the Interdisciplinary Group/Team, document progress, and participate in on-call coverage. Travel locally and adapt to home environments are required for this role.

Qualifications

• Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse (RN).
• Two years RN experience in a clinical care setting; hospice preferred.
• Good interpersonal skills; read/write English.

Responsibilities

• Initial and ongoing comprehensive assessments of the impact of terminal disease on patients' physical, functional, psychosocial, and environmental needs.
• Develop and implement plan of care to ensure quality and continuity of care; educate patient and family.
• Coordinate with the Interdisciplinary Group/Team and provide training to staff as needed.
• Perform in-home visits; provide on-call responsibilities and timely documentation.
• Ensure infection control and standard precautions; monitor care quality and outcomes.
