Job summary

Community Hospice and Palliative Care in Jacksonville, FL seeks an RN Case Manager to plan and manage nursing care for an assigned caseload in home or long-term care settings. Qualifications

• Proven knowledge of hospice and palliative nursing principles.
• Ability to educate patients and caregivers about disease progression, medications, nutrition, and hospice philosophy.
• Strong communication skills in verbal, written, and non-verbal forms.
• Must maintain professional boundaries and resolve team conflicts.

Responsibilities

• Assess and identify patient and family goals and needs.
• Collaborate with interdisciplinary team to develop and implement individualized care plans.
• Participate in team meetings and ethical discussions.
• Educate patients and caregivers about disease progression, medications, nutrition, and hospice philosophy.
• Coordinate timely and effective responses to changing or urgent patient needs.
• Maintain accurate, timely, and compliant documentation.
• Support and educate team members, patients, families, and the broader community.
• Participate in internal quality improvement initiatives.
• Facilitate interdisciplinary group meetings in the absence of the manager.
• Perform other duties as assigned.
