UnitedHealth Group is seeking a Hospice Registered Nurse to provide patient-centered care in home-based settings. You will assess needs, coordinate individualized care plans, manage symptoms, educate families, and collaborate with the clinical team.

Responsibilities include admitting new patients, revising care plans, communicating progress to the team, and maintaining accurate care records. The position involves on-call duties and requires an active RN license and CPR certification.

Qualifications

• Current RN license in the state(s) you work.
• Valid driver’s license, reliable transportation, and liability insurance.
• Current CPR certification.

Responsabilités

• Performs initial nursing assessment on new admissions.
• Coordinates and revises the plan of care with the clinical team.
• Communicates patient progress to the clinical manager and care team.
• Maintains individualized patient care records accurately and timely.
• Records pain and symptom management changes and outcomes.
• Educates families on patient condition and care needs.
• Makes referrals to other hospice care disciplines as needed.
• Performs on-call responsibilities and on-call services.
