Job summary

Lifespark is seeking a Hospice RN Case Manager to plan, organize, and direct compassionate hospice care in patients' homes and communities. The RN will collaborate with families and professional staff, performing comprehensive assessments and coordinating the interdisciplinary plan of care.

Responsibilities include patient education, medication administration as prescribed, and ensuring quality patient outcomes.

Qualifications

• 2 years’ RN experience in health care (preferably home care or hospice).
• Experience supervising caregivers.
• Knowledge of corporate regulations, ACHC standards, and payer practices.

Responsibilities

• Conduct initial and ongoing patient/family assessments to determine hospice needs and plan of care.
• Provide professional nursing care per nursing process and act within state Nurse Practice Act.
• Initiate and revise the plan of care as patient needs change and coordinate with the interdisciplinary team.
