Job summary

Lifespark is seeking a Hospice RN Case Manager to plan, organize and direct hospice care across home and community settings, leveraging strong nursing expertise to support patients and families. The role emphasizes collaboration with an interdisciplinary team and community health resources to provide compassionate, comprehensive care.

The position is a 1.0 FTE role in the Northwest Metro area (Buffalo region) with mileage reimbursement and a focus on quality, safety, and education for patients

Qualifications

• 2 years’ RN experience in health care (preferably home care or hospice).
• Experience in customer service/direct member contact field preferred.
• Experience supervising caregivers.
• Knowledge of corporate business management, governmental regulations, ACHC standards, and private payer practice.

Responsibilities

• Conducts initial, comprehensive assessments of patient and family to determine hospice needs.
• Provides professional nursing care per nursing process and state acts.
• Develops and revises plans of care with patient/family involvement.
• Initiates and revises the care plan as patient needs change.
• Coordinates with physicians and the interdisciplinary team to ensure continuity of care.
• Supervises ancillary personnel and delegates as needed.
• Maintains accurate clinical notes and documents changes in symptoms.
• Supports patient and family with education and self-care techniques.
