Gentiva Hospice in Abilene, TX is seeking a dedicated Registered Nurse (RN) to provide compassionate hospice and palliative care in patients' homes and community settings. You will assess, plan, and deliver skilled nursing care in the home, adjust care as patient needs change, and collaborate with an interdisciplinary team to ensure comfort and dignity.

Qualifications

• RN licensed in the state of employment (Texas).
• Minimum of 1 year RN experience in hospice, home health, or related setting.
• Current CPR certification.
• Valid driver's license and auto insurance.
• Knowledge of hospice and palliative care principles.

Responsibilities

• Be the primary point of contact for patients and families, guiding them through the hospice journey.
• Deliver and document skilled nursing care based on each patient’s Plan of Care in home or community settings.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with an interdisciplinary team to refine care plans.
• Provide education, support, and bereavement guidance to families.
• Maintain timely clinical records and follow infection control standards.
• Participate in on-call and patient support activities as applicable.
