Job summary

Eden Health Hospice in Reno, NV seeks an experienced Registered Nurse to serve as an RN Case Manager in a 100% employee-owned hospice. You will coordinate patient plans, deliver expert nursing care, and liaise with physicians to ensure seamless care from admission to discharge.

The role emphasizes compassionate, patient-centered care, collaboration with the interdisciplinary team, and adherence to hospice standards and safety practices.

Qualifications

• Graduate of an accredited nursing program.
• Current licensed RN in the state of practice.
• One year of Hospice experience within the last 24 months.
• Valid driver’s license and reliable transportation.
• Proof of automobile insurance (MVR checked).
• Proof of CPR certification.
• Able to pass criminal background check and national sex offender clearance.

Responsibilities

• Coordinates the total plan of care and maintains continuity of patient care with the interdisciplinary team.
• Acts as Case Manager from admission through discharge for hospice patients.
• Participates in patient care conferences and educator of patient/family on rights and responsibilities.
• Communicates changes in condition to Attending Physician and Hospice Medical Director.
• Ensures resources are utilized equitably to meet patient needs.
