Job summary

St. Joseph Hospice in New Orleans, LA seeks a Full-Time Registered Nurse (RN) to provide compassionate hospice care in patients' homes and facilities. The role includes initial and ongoing patient assessments and implementing the Hospice Plan of Care.

Ideal candidates hold Louisiana RN licensure, CPR, EMR familiarity, and 2+ years of RN experience in hospice, oncology, or home health. A valid driver’s license with auto insurance is required; ESOP benefits are offered.

Qualifications

• Louisiana RN license required.
• Minimum 2 years full-time RN experience in hospice, oncology, acute, home health, or LTC.
• CPR certification required.
• Valid driver’s license and auto insurance.
• EMR knowledge required.
• Home Health or Hospice experience preferred.

Responsibilities

• Performs the initial and ongoing assessment of the patient.
• Assist in developing and implementing the Hospice Plan of Care.
• Educate patient and family on Plan of Care, including medication administration and care details.
• Act as primary liaison between physician, patient/family and hospice team.
• Maintain an accurate and updated medical record.
• Makes routine supervisory visits.
• Participates in agency call schedule rotation.
• Adhere to policies, regulations and reporting requirements.
