Job summary

Empyrean Hospice is seeking a Registered Nurse Case Manager to plan, organize, and direct hospice care with emphasis on community health education and experience.

The role requires current RN licensure, CPR, and nursing experience. You will assess patients, develop care plans with families, coordinate services, and educate families while maintaining comprehensive documentation and collaboration with the care team.

Qualifications

• Professionalism, compassion, empathy, and a desire to help others.
• Self-directing with ability to work with little direct supervision, yet also as member of an interdisciplinary care team.
• Understand the philosophy of hospice care.
• Current licensure in State and CPR certification.
• Graduate of an accredited school of nursing with 1-2 years of recent acute care experience and 1-2 years in home care.
• Bachelor's degree, with hospice care experience preferred.

Responsibilities

• Represent Empyrean Hospice with professionalism and compassion.
• Complete initial and ongoing patient/family assessments to determine hospice needs.
• Develop and implement individual care plans with patient and family involvement.
• Coordinate patient care for assigned caseload.
• Counsel patients and families and provide care instructions as appropriate.
• Assist with activities of daily living and promote patient self-sufficiency.
• Maintain and submit accurate clinical notes on patient condition and care.
• Communicate with physician and hospice team to ensure continuity of care.
