Job summary

Eden Health Hospice in Longview, WA is seeking a dedicated Registered Nurse to serve as a Case Manager from admission through discharge, coordinating care with the interdisciplinary team to ensure compassionate, high‑quality end‑of‑life support. You will assess patient needs, educate families, manage the plan of care, and advocate for comfort and dignity while handling caseloads of 12–18 patients.

Qualifications

• Graduate of an accredited nursing program.
• Current licensed RN in the state of practice.
• Experience as an RN with one of the following preferred: one year in acute-care, home health care, community health, or hospice in the last 24 months.

Responsibilities

• Provide hospice nursing care from admission through discharge.
• Coordinate with interdisciplinary team to ensure continuity of care.
• Educate the patient and family about the plan of care and expectations.
